adapt content-negotiation method combination

Make CALL-NEXT-METHOD cause an irrevocable control transfer: returning
from any primary method returns from all of them.  (Because
media-types are basically disjoint, it doesn't really make sense to
have CALL-NEXT-METHOD return).
